Not on a spaceframe, it was on a twin. Keith, your first option is my guess too. And yes the hubs were newly manufactured.
 
...I`d hazard a guess and say this is the sort of catastrophie that would happen..
                                                                                      if someone had assembled the spoked wheel hubs with the bearing shims that are used in the cast wheels.
